Quick answer

Rate a course on content, pacing, materials, assessment, support and value, then read the pattern per cohort to decide the two changes worth making next.

💡

About this template

An overall happiness score hides the decision. Satisfaction only becomes useful when it is split: was it the content, the pace, the materials, the assessment load, the support or the price? This survey rates those six dimensions and asks two short open questions - start and stop - so the result points at a specific change. Because responses land in one place, you can read a cohort, a session or a facilitator side by side instead of averaging everything into a number that no one can act on.

Frequently Asked Questions

Is this the same as an NPS survey?
No. NPS measures likelihood to recommend; this measures which parts of the course worked. Run both and you know whether a good course is also one people would bring a colleague to.
Six dimensions or ten?
Six is enough to decide. More dimensions lower completion and blur the read.
Should we ask at the end of the course?
Ask mid-course and at the end. The end round alone means the current cohort absorbs the problems.
Can it be anonymous?
Yes - drop the identity field. Keep the cohort field so results stay groupable.
Does it generate a report?
Not this one: it is a collection survey. Use the assessment templates with an AI report where each learner needs a written result.
Can we run this survey again for the next cohort?
Yes. Copy it into your FormLM workspace, set dimensions, and reuse it each cohort.
