Quick answer

Training ROI stalls at 'were you happy' — score reaction, learning, behaviour and results each on anchored scales, and the AI report shows exactly where the chain breaks.

💡

About this template

Happy-sheet scores don't prove impact. This evaluation grades a program across all four Kirkpatrick levels — did people engage, did they learn, did behavior change at work, did results move — and the AI report shows where the chain breaks, so you fix the level that actually leaks value instead of re-running the same liked-but-useless course.

❓

Frequently Asked Questions

Do you really need all four levels for every course?
Not for every course — but when someone asks training ROI, the answer sits in the level where the chain breaks. Running all four once per program makes that break point visible.
When do you measure behavior and results?
Reaction and learning right after the session; behavior around 30 days via the participant or their manager; results at the next KPI cycle the program claims to support.
How is this different from the Training Feedback Form?
The feedback form captures the reaction layer only. This evaluation extends the chain through learning, behavior change and business results, and shows where value leaks out.
And versus a training pre/post test?
The pre/post test is evidence for the learning level alone; here it slots into the full chain — knowing scores rose is different from knowing behavior changed.
Is it free to use?
Yes. Copy it into your FormLM workspace and name the program and KPI you evaluate.
Does it work for one trainer or a whole class?
Both — each response generates a personal report, and collected responses show level distributions across the class.
