Quick answer

Self-reported attribution asks three things analytics cannot answer: where you were first discovered, where you were last checked before buying, and what in the customer's own words tipped the decision. Because untracked touches — word of mouth, podcasts, printed material, unlinked PDFs — land in "direct" traffic, reported sources commonly reassign 30–50% of conversions. Keep the survey to under a minute at the moment of purchase, then compare it with your dashboard monthly instead of trying to make one replace the other.

💡

About this template

A channel budget is usually argued from the same dashboard, so the same blind spot gets funded every quarter: the touch that created awareness is invisible, and the touch that closed the sale takes all the credit. This form splits first touch from last touch, records the medium in the customer's own language, and adds one question about what they compared you against — enough to see whether a channel is generating demand or merely harvesting it.

Placement matters more than length. Put it on the thank-you page, in the order-confirmation email, or as the first two questions a sales development rep asks on a qualified call. If the competitive read matters more than the discovery path — who you were weighed against and what decided it — use the Win/Loss Comparison Survey instead, and run this one on every customer.

Frequently Asked Questions

Why ask "how did you hear about us" if analytics tracks sources?
Analytics sees only what it can tag. Direct traffic absorbs podcasts, word of mouth, offline conversation and unlinked documents, and last-click credits whichever page preceded purchase. Self-reported attribution routinely moves 30–50% of conversions out of "direct" — which is where the highest-intent touches hide.
Should the survey run at purchase or after it?
At purchase for short cycles — a thank-you page or order confirmation catches memory while fresh, usually within one or two weeks. In long B2B cycles, ask at the first sales call and again at close, because the triggering touch and the convincing touch are often different.
How long should an attribution survey be?
Three to five questions, under a minute. Past that, respondents choose the first option to finish and the free-text answer — the part that cannot be faked — gets skipped.
What do I do with contradictory answers?
Keep them. Contradiction between reported and attributed source is the finding: it usually marks a content or community touch that started consideration and was credited to brand search at the end. Compare both data sets monthly instead of crowning a winner.
How is this different from a win/loss survey?
A win/loss survey interrogates one decision you won or lost against named competitors and the tie-breaker reason. This maps the discovery path across all customers, including those who never compared.
Is it free?
Yes. One click copies the form and its logic into your FormLM workspace — edit the channel list to match the sources you actually buy.
