Let the people you serve grade the service itself — access, treatment, usefulness — in two minutes, on their phone, without a paper comment box.

Quick answer

Use it after a service contact to measure what people actually experienced — access, how they were treated, usefulness — in about two minutes, anonymously if they choose. Ratings plus one open change question give staff a fixable list, not a vanity score.

💡

About this template

The people best placed to judge a service are the ones who had to use it. This survey keeps their time honest — two minutes on a phone — and their answers useful: ratings on access, staff treatment and usefulness, plus one open line about what to change. Dignity rules the wording: no five-point mazes, no mandatory identity.

❓

Frequently Asked Questions

Why measure satisfaction separately from outcomes?
Because they can disagree: a program can change lives and still treat people badly, or delight them while achieving nothing. Outcomes grade the situation; satisfaction grades the service experience — you need both lines.
Can people answer anonymously?
Yes — identity is optional. Program and timing tags carry enough structure for trend analysis.
Why ask away from staff?
Face-to-face feedback skews polite. The link goes out at follow-up distance, not at the counter.
Is it free to use?
Yes. Copy it into your FormLM workspace, set dimensions to your service stages, reuse each wave.
How short is short enough?
Two minutes, phone-first. Past that, the people least able to spend time drop out — and they are your signal.
What do we do with a bad quarter?
Three separate lines make the failing dimension visible — access, treatment or usefulness — so the fix targets one thing, not the whole program.
