Quick answer

Use it to enroll, not just register: collect background, expectations, access needs and availability per participant, assign cohorts as seats fill, and capture guardian confirmation where the program serves minors.

💡

About this template

Enrolling a program is different from registering an attendee: you need to know who fits, what they need from it, and whether it's the right cohort — before the first session, not at the door. This sign-up collects application-grade detail per participant — background, expectations, access needs, availability — assigns cohorts as seats fill, and carries guardian confirmation when the program serves minors.

❓

Frequently Asked Questions

When should you recruit per program instead of per event?
Whenever continuity is the product — courses, clubs, support circles. Event sign-ups count a body once; program enrollment must place a person across a whole season.
Is this the same as charity event sign-up?
No — events admit and seat; programs admit into a commitment. The motivation and commitment fields exist to protect the cohort's continuity.
What detail should you not collect?
Skip medical records, IDs and finances. Access needs with consent and honest experience level are enough for placement.
Is it free to use?
Yes. Copy it into your FormLM workspace, define cohorts and seats, and run one round per intake season.
How do waitlists work here?
Cohort seats close automatically; the waitlist keeps its order and converts as seats open.
Can someone leave mid-program?
Yes — freeing the seat updates the cohort count, and the next waitlisted person gets the offer.
