Quick answer

Nice diversity numbers do not mean everyone is treated well — six inclusion dimensions scored anonymously with group thresholds before any reporting, and the slices show where inclusion actually breaks.

💡

About this template

Diversity numbers show who is in the room; inclusion surveys show whether they can thrive there. This survey asks employees to score belonging, respect, fairness, voice, psychological safety and growth access — anonymously, and sliceable by team — so the results show exactly where inclusion breaks down instead of a company-wide average nobody believes.

❓

Frequently Asked Questions

Is it safe to run a DEI survey anonymously?
Yes, with reporting thresholds — only publish groups above a minimum size (commonly 5–10) so no individual answer is readable.
Should we ask demographic questions?
Opt-in, broad and at the end. Without some slicing you get an average that hides exactly the gaps you are looking for.
How often should we run it?
Half-yearly works; inclusion moves slower than morale. Identical items keep the trend honest.
What if the scores reveal uncomfortable gaps?
That is the point — publish the findings and the specific response, or the next survey collects the polite answers.
How is this different from an engagement survey?
Engagement asks how energized people are; this asks whether everyone, across groups, is treated and heard equally.
Is it free to use?
Yes. Copy it into your FormLM workspace and adjust the dimensions and thresholds.
