About this template

Long satisfaction surveys fail because clients read them as homework and answer them as diplomacy. This debrief stays short on purpose and anchors every question to a specific finished piece of work: name the project in the intro, ask about the outcome rather than "the service", and force a reason behind the recommend score so a 9 cannot hide an unresolved complaint. The final two open questions are where renewal risk usually surfaces — usually about responsiveness or scope creep, almost never about the quality of the deliverable.

When should a client feedback form be sent?
Within a week of delivery, while specifics are still rememberable and before the next commercial conversation. Sending at renewal biases answers toward what the client hopes to gain; sending months later produces polite generalities.
How many questions before response rates collapse?
For a paying client, six to eight is the practical ceiling. Past that, completion drops sharply and free-text quality drops with it — this form keeps to six so the open questions actually get written.
What is a good response rate for client feedback?
A named form sent by the account owner typically returns 25–40%. Below that, the usual cause is that nobody tells the client what their answers will change — so say so in the invitation.
How is this different from a client satisfaction survey?
A satisfaction survey audits the whole relationship — strategy, execution, reporting, communication — usually quarterly. This debrief grades one finished engagement in two minutes, which is why completion is far higher.
Can I ask for a testimonial in the same form?
