Quick answer

A testimonial is personal data and often commercial information, so "they loved the project" is not a licence to publish. This form attaches the exact approved wording to a permission grant: attribution style (full name and title, first name only, or anonymous), the channels covered (website, paid ads, case study, social, print, sales decks), whether you may edit for length, whether results figures are cleared, and a validity period with a withdrawal contact. The timestamped response is the evidence that the asset was approved in that form.

💡

About this template

Consent requests fail for one reason: they are vague. A blanket "may we use your feedback?" gets a yes that means the website, and then a lawyer's letter when the quote appears in a paid campaign beside a competitor's logo. This form splits permission by channel and by attribution style so customers approve what they are actually comfortable with — which raises approval rates rather than lowering them, because saying yes to two things is easier than saying yes to everything.

Send it with the draft in hand: paste the exact sentence and any figures you intend to publish, so the approval is specific and auditable. Collect it straight after a strong debrief — the Client Feedback Form asks the same "what was the result?" question and produces the raw material. For customer-created photos, video and reviews, use the UGC Submission Form instead, which clears rights over media rather than over a written quote.

❓

Frequently Asked Questions

Do I need written consent to use a customer testimonial?
In practice yes. A testimonial usually contains the customer's name, company and sometimes commercial results — personal and confidential information used for advertising. Written consent defines what you may publish, where, and whether figures were approved, which is what protects you if their employer objects later.
What should a testimonial release form include?
Five things: the exact approved text or media, the permitted attribution style, the channels covered, whether you may edit for length, and a validity period with a withdrawal route. Vague "we may use your feedback" wording is the one that causes problems.
Can a customer withdraw consent after publication?
Yes, and the form should say how. Consent to use personal data can generally be withdrawn, and publishing without it is not defensible. A common compromise is a stated review window plus removal of future use, since recalling printed material retroactively is not always possible.
Is a typed name a valid signature?
For ordinary marketing consent, an electronic acceptance — typed name, timestamp and the record of exactly which wording was approved — is normally sufficient and far easier to collect. Regulated industries and B2B confidentiality agreements may require a formal signature; take local legal advice there.
How is this different from a UGC submission form?
A UGC form collects customer-created photos, videos or reviews together with rights to republish that media. This form clears rights over a quote or story about a business relationship, including whether commercial results may be named.
Is it free?
Yes. One click copies the form into your FormLM workspace at no cost — adjust the permission lines to match your contracts.
