Should I use a 5-point or 7-point Likert scale?

Five points is the workhorse: reliable, familiar, easy to report. Go 7-point when you need more spread at the top end; go 4-point when you want to force a lean.

Is a neutral midpoint good or bad?

It reduces forced answers and measurement error, but invites fence-sitting. Drop it (even-point scales) when you need a directional decision from every respondent.

How should anchors be worded?

One dimension per scale (never mix frequency with quality), plain words, symmetric around the midpoint, and the same order in every item.

Where do the numeric codes come from?

Scales are coded 1..N for analysis; reverse-score negative-worded items before averaging—FormLM handles reverse scoring in its scoring model automatically.
