💡

About this template

Teams do not lack ideas; they lack people willing to spend social capital on them. This instrument scores the six recognised safety dimensions - speaking up, admitting mistakes, asking for help, challenging the decision, being different and taking risk - then compares individual responses to the team mean. The report shows which dimension is the constraint and where the gap between leaders and members is widest.

Frequently Asked Questions

What is psychological safety?
A shared belief that the team is safe for speaking up, taking risk and admitting error - measured as team perception, not individual personality.
Is answering anonymous?
Responses are collected per person and reported as team aggregates, so individual answers are not exposed.
How is this different from engagement?
Engagement measures commitment; safety measures whether people will tell you the truth. Teams can be engaged and silent.
What does the report give me?
The lowest dimension, the leader-member gap and one behaviour change to try for a quarter.
How often should it run?
Quarterly. Safety moves slowly, and re-running too soon just measures noise.
Can I use it one team at a time?
Yes, and that is recommended; comparing teams is useful, ranking them is not.
